The first non-surgical step usually involves Scaling and Root planing ("Deep Cleaning"). The objective of this conventional therapy is to remove etiologic agents which cause inflammation to the gingival (gum) tissue and surrounding bone. Common etiologic agent include dental plaque and tartar (calculus).

After the treatment is complete, the majority of patients will require ongoing maintenance therapy to sustain health. Non-surgical therapy does have its limitations; however, and when it does not achieve periodontal health, surgery may be indicated.
